Team Entry Packets

Team entry packets will be available for coaches to acquire from our office beginning Tuesday, January 19, 2010.  Team packets will contain a team roster form, individual player registration forms and parent waivers.

Team entries are due to the Kids, Incorporated office no later than 5:30pm, Wednesday, February 3, 2009.  Team entries should include the following:

• payment for at least seven (7) players

• parent waivers for at least seven (7) players

• completed team roster form
